Looking for:
preloneFor the treatment of mild to moderate laryngotracheobronchitis †. For the treatment of exacerbations of chronic obstructive pulmonary disease †. For topical ophthalmic treatment of corticosteroid-responsive eye disorders.
The initial dosage of PRELONE (prednisolone ) Syrup may vary from 5 mg to 60 mg per day depending on the specific disease entity being treated. In situations of less severity lower doses will generally suffice while in selected patients higher initial doses may be required. The initial dosage should be maintained or adjusted until a satisfactory response is noted. If after a reasonable period of time there is a lack of satisfactory clinical response, PRELONE (prednisolone ) Syrup should be discontinued and the patient transferred to other appropriate therapy. Systemic corticosteroids, such as prednisolone, may decrease glucose tolerance, produce hyperglycemia, and aggravate or precipitate diabetes mellitus. When corticosteroid therapy is necessary for patients with diabetes mellitus, changes in insulin, oral antidiabetic agent dosage, and/or diet may be required.
Advise a pregnant woman about the reproductive risk and the potential harm to a fetus. Neonates born to pregnant women who have received corticosteroids should be carefully monitored for signs and symptoms of hypoadrenalism, and appropriate therapy should be initiated, if necessary. Ophthalmic prednisolone and other ocular corticosteroids were applied to both eyes of pregnant mice ; a significant increase in the incidence of cleft palate was observed in the fetuses of the treated mice. As sodium retention with resultant edema and potassium loss may occur in patients receiving systemic corticosteroids, these agents should be used with caution in patients with congestive heart failure, hypertension, or renal disease or insufficiency.
Therefore if using prednisolone as treatment for croup a second dose is recommended. Prednisolone is administered orally, while prednisolone sodium phosphate may be administered orally or by IM, IV, intra-articular, intrasynovial, intralesional, or soft tissue injection or by IV infusion. The prednisolone acetate preparation may be administered by IM, intra-articular, or soft tissue injection. Prednisolone tebutate is administered by intra-articular, intralesional, or soft tissue injection.Any prednisolone that is systemically absorbed is distributed into the kidney, muscle, liver, intestines, and skin tissues. The circulating drug is 70—90% bound to plasma proteins, and only the unbound portion of the drug is active. Corticosteroids distribute into breast milk and cross the placenta.
The onset is usually acute with a few hours of high fever, lethargy, soft inspiratory stridor and rapidly worsening respiratory distress. The child appears toxic with a high fever and is often sitting immobile with their chin slightly lifted and mouth open drooling saliva. Attempts to lie the child down or painful procedures can precipitate complete upper airway obstruction and so should ideally only be done when the airway has been secured. Treatment involves urgent PICU review for airway assessment and management, bloods for culture and IV cefotaxime. A single dose Prednisolone (1mg/kg) is not as effective as a single dose of dexamethasone for the treatment of croup.
0.14 to 2 mg/kg/day PO or 4 to 60 mg/m2/day PO per day, given in 3 to 4 divided doses. Individualize dosage to patients condition and treatment response. 25 Although adverse effects of nebulized epinephrine are rare, patients receiving frequent treatments should be monitored for adverse cardiac effects.
I tasted a tiny bit of it myself, it was awful and I could immediately tell that it had alcohol in it. I discussed this with the pharmacist who indicated the medicine could be "hidden" in juice etc. Unfortunately, the flavor of the prednisolone is overpowering the taste of the apple juice I mixed it with. She is slowly getting through it, but I hope someone might have a better idea. Your child may find it difficult to drink fluids as a result of an obstruction in their airway. However, it is important they have plenty of fluids to avoid dehydration.
